I wonder if I have to make the image separately for draft/export.
Is there a way to export without a shadow effect?
Or in practice, do designer have to create a separate extraction image
that removes the shadow next to the draft?

This isn’t currently possible, but it’s already on our team’s radar for improvement. You can add your feedback and vote for this capability here:
Currently, an image with an effect, like a drop shadow, is exported with that effect. If a developer wants the image without the shadow, to add with CSS separately, they must disable it in the design, export, then turn it back on. This is cumbersome and prone to error. Instead, there should be another checkbox in the export menu allowing this to be done there, without altering the design.

As you suggested, you can create a separate image without the shadow effect as a workaround.
