It’s more consistent than it was years ago, but still: so often I have to Restore default Thumbnail → Set as Thumbnail, then refresh my browser/app on the Files screen to see if it worked. Often it takes 3 to 4 tries for some files

The team confirmed that this was a bug with thumbnails not being refreshed at a consistent cadence on the backend, but as you already mentioned, after some time, refreshing should fix it.
As a rule of thumb, it can take up to a minute for thumbnail changes to be reflected, so I would wait 60 seconds… (maybe 90 seconds to be sure) before refreshing to see if the thumbnail changes have been implemented.
