Changes to components should always carry over to instances

When a change is made to an instance, any updates made to the original component after that do not carry over. This is very frustrating and results in me updating each instance.

Yes. Changes to components used to propagate to their instances (unless a particular aspect of the instance was overridden). But now Figma doesn’t seem to update instances at all, or what does get updated is very haphazard. Aside from requiring way more work, and not taking advantage of one of the main benefits of the parent-child relationship (updates), this causes huge problems for an organization. People are using the wrong versions of components; at best, they’re confused by the discrepancy, at worst, they’re pushing the wrong version into live code.
