In a lot of new documents that I create I see this update from two years ago. Why?

We’re seeing a similar thing on files that have had all updates applied. Frequently (multiple times an hour) random updates from months ago will appear. This is making it very difficult to track when files have valid updates because the update flag is almost always on.
